I was planning on spendng some time writing such a plug-in, but it seems that someone else did the job for me - Wordpress Twitter Updater.
Installs just like any other plugin. The admin screen allows you to enter your Twitter account details, of course. Additional configuration determines when a tweet is posted: when you publish a post, when you edit a post, or even when you are writing a draft post. Each can be switched on or off, and you can assign your own custom prefix to the post title/link which is automatically published to Twitter.
If it works, then this very post will be the first one to appear in my Twitter stream. If it doesn’t then this post will look rather daft!
UPDATE: Arse! Didn’t work. Researchng, will report back.

Hi Neil, I think James Cridland published a fix for this on his blog - if it’s the plugin I’m thinking about. All the best, Ian.
